In the picture the sac looks full,a buck that has been hit with a burdizzo will have deflated sacs.

Cattle also have been known to retain a testicle up in the abdominal cavity. Called stags. Sounds to me that they missed one nut. It can descend if there is any room and it looks like yours has enough room in the sack. Sometimes it is hard to find it if it is retained up inside, but the vet should have been able to figure that out.

A new vet may be in order. They dont seem to know a lot about goats.
