I agree that your problem was probably that the mother has no idea what she is doing with room to move around. I worked on hog confinement farms in college. I worked in the farrowing dept. Previous poster is right that they have little room to move and less room to squish them. She has no idea about protecting her babies. The 2 you lost that you said were no where to be found. She probably ate them. They will do that some times, especially if they are in pain or sore from the birth.
