My middle granddaughter once asked me what were those lines in my face. I told her they were wrinkles. In horror she asked where did they come from. I told her I had a lot of birthdays, have a birthday, get a wrinkle. She thought for a moment then said, I don’t want any more birthdays. 😃😃😃😃

My only thought on the ivermectin is that we use a pour on, on the cattle... which helps to control worms in their gut tract.. so it has to permeate down through the skin into tissue that carries it throughout the body. There are lots of cattle people and vets that say it is best for parasites like lice ticks etc... and not effective on internal parasites...ie... worms.... BUT... I tend to disagree. We had some cattle that had dropped alot of weight last winter.. some were really doing poorly... used the headlocks on the silage wagon and got all the cattle locked in and I took fecal samples from the 2 worst looking cows, 2 from a couple of average looking ones, and 2 from 2 fairly fat cows... The 2 worst looking one's numbers were specifically written down... the other 4 were just random of who I could easily get to stand to get samples with the ob glove. We then used pour on wormer on every cow since they were contained in the headlocks to make sure that they all got a dose.
Took the samples to the vet to run fecals. Results were 2 very high counts, 2 medium counts and 2 counts of next to no worms...

Took samples again, about 3-4 weeks later... and made sure I got the 2 "worst looking ones" again.... since I had their numbers... and 4 other random cows.
They ALL came back with next to NO problems... besides the cows were looking 10 x better than a few weeks earlier.
That is to me some proof that alot of the naysayers are wrong. It was a strictly one time "test" on our animals... we had changed nothing except the pour on ivermectin when we caught them up that day.

We do NOT routinely worm our adult cows... and have sold a few that seemed to need worming more frequently. They also tended to not raise more than mediocre calves.
All that said, I don't believe it is just surface at least not on everything. Maybe a little more time before you do any surgery if you are not ready to do it???? Do they say there are any negatives to waiting a little longer? I am NOT suggesting you neglect your health or the situation... Just asking a question... How long have you been using it to have had the results on the surface already?

I'm with @farmerjan on this. In the essential oil world I learned that anything applied topically to the skin gets into the bloodstream, I want to say in 20 seconds? but truly can't remember the exact number. Which is why I cringe when I see all the commercials that push scented laundry products and room fresheners -- and you know they are just using chemicals and not the actual essential oils to get those products. (quietly sliding that soapbox to the side).
